What are Care Managers?

A Professional Geriatric Care Manager (PGCM) is a health and human services specialist who helps families who are caring for older relatives. The PGCM is trained and experienced in any of several fields related to care management, including, but not limited to nursing, counseling, gerontology, or social work, with a specialized focus on issues related to aging and elder care.

Geriatric care management usually involves an in-depth assessment, developing a care plan, arranging for services, and following up or monitoring care. Families are not obligated to implement any part of the suggested care plan. However, due to their experience and familiarity with community resources, PGCMs often suggest potential alternatives the family may not have considered. PGCMs can also ensure the aging person receives the best possible care and any benefits to which they are entitled.

Utilizing a Professional Geriatric Care Manager typically saves the family money in the long run, by assessing precise needs and helping the client and family choose the most appropriate specific service. Following the initial assessment, the PGCM will help the family carefully estimate the ongoing cost of service delivery.
