Life-Links listens to your concerns about your loved one’s living situation to better understand what you want and what you need for your aging loved one.
Life-Links helps you decide whether our senior services are a good fit for you or if a simpler, more cost-effective solution is available. There is no charge for this free initial phone interview and we will answer any questions you may have about our pricing. Call us at (615) 595-8929. If there’s a fit, we’ll review paperwork and obtain signatures consenting to our services and our HIPAA release forms.
Life-Links assesses the living situation in person. Two Life-Links senior care managers will meet with the client and a caregiver to discover an accurate and holistic view of the individual and an in-depth understanding of current challenges.
Life-Links develops a plan of senior care management and services based on the individualized needs identified at the on-site assessment. We will outline precise, recommended actions and work together to determine who will provide these services and determine budgetary guidelines. We will consider all family, community and professional resources available. Our recommendations include, yet are not limited to:
- living options (independent living, assisted living, nursing home, health care centers)
- plan of care
- advance directives and other necessary paperwork
- accessing benefits such as Veteran’s Aid and Attendance or long term care insurance
- coordinating home health (medical and non-medical and/or sitters)
- home safety
- assistance with activities of daily living
- medical options
- bill organizing and paying
- transportation
- medical equipment options
Life-Links implements the plan entirely, not at all or by dividing tasks with you. It’s your choice.
Life-Links monitors the senior care management and services being provided and reassesses your loved one and the plan, as necessary.
What are some warning signs it’s time to get help?
You’ve noticed the following changes about your aging loved one:
- Frequent falls, with or without injury
- Significant weight loss or gain
- Multiple doctors and doctor appointments, and they don’t seem to be talking to each other
- Frequent ER visits
- Does not take medications as prescribed
- Confused, forgetful, anxious or depressed
- Clothing or home is dirty, refuses to bathe
- Unable to pay bills or do paperwork
- Abusing alcohol or medications

How does Life-Links address confidentiality of my family’s situation?
Life-Links makes every effort to keep your personal information confidential. We understand that over time, family secrets may be revealed or your loved one may divulge confidential information or feelings to us they do not want shared. Legally, we are bound by law to report any incidences of abuse, neglect, and exploitation to the authorities. Ethically, we make every effort to only release absolutely necessary information during the case management relationship so that our client gets the care and services they need, without other people knowing “their business.” This is extremely important as the foundation of our success with our clients is built on the trust they place in us.
How much does it cost?
We charge a flat fee for the initial assessment and ongoing services are charged at an hourly rate based upon the customized plan of care we create with you. Most of our families find costs to decrease as we become more involved and the situation stabilizes. Utilizing a Professional Geriatric Care Manager typically saves the family money in the long run, by assessing precise needs and helping the client and family choose the most appropriate specific service.
